CDRP Submission Forms

If, as a First Nation Class Member, you have delivered to ISC under Section 9.06 of the Settlement a written notice and a Remediation Plan and:

  1. Ninety days have passed since delivering the written notice and Remediation Plan and you have determined that ISC has not met its Commitment under the Settlement; or
  2. You have determined at any time that ISC is not complying with a Remediation Plan that it agreed to implement; 

then you may seek to resolve this dispute through the CDRP.

The first step of this process is to complete and deliver this Collaborative Negotiations Notice to ISC, thereby formally triggering the CDRP (If Collaborative Negotiations fail to resolve the dispute, a similar Mediation Notice should be delivered to ISC. If Mediation fails to resolve the dispute a similar Arbitration Notice should be delivered to ISC.)
